Primary Care Network
We are a member of Unity Primary Care Network (PNC). This means we will be working closely with a number of other practices and other health and care organisations to provide healthcare services to you.
During this course of our work we may share your information with these practices and health care organisations/professionals. We will only share this information where it relates to your direct healthcare needs. When we do this we will always ensure that appropriate agreements are in place to protect your information and keep it safe and secure. This is also what the law requires us to do.

New Patient Registration
Patients living within the practice area are welcome to register and our reception staff can confirm your eligibility from your postcode.
It will speed up the process if you bring along the following when you register:
- NHS number - as shown on your medical card (or this can be obtained by contacting your previous Doctors)
- Previous Doctors name, address and postcode
- Photographic ID (passport or driving licence) and a utility bill for proof of address; utility bills should be no more than 1 month out of date (E.G. Registering in March - utility bill should fall into February's date)
If you would like on line access (to make appointments, order prescriptions and see part of your medical record) then you will need to bring along the following:
- In date, photographic primary identification. This can either be your passport or UK driving licence.
You will need to complete a registration form (GMS1) and a medical questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. New patients are offered a health check and the opportunity to have an HIV blood test with a member of the healthcare team.
Medical treatment is available from the date of registration. Please contact reception for further information.
